Year-Round MS Math Tutoring

I understand that during the course of the IBA program, pictures are taken that are often used in our yearbooks, website, etc. I give my permission for my child's picture to be used by the school.

For all cancellations or refunds, please contact your respective center.
After-School Programs: Non-refundable after the first lesson day.
Summer Camps and Holiday Camps (including Winter Break and Spring Break): Refundable only up to one month before the first day of camp. After this period, only tuition credits are offered.
Year-Round Programs: Please register at least 5 days before the first class. We require a 24-hour advance notice for class absences to schedule a make-up class. Make-up classes must be completed within the same month and cannot be carried over to the next month. To discontinue lessons, one month's advance notice is required.
Tournaments: NO REFUNDS.
Credit Requests: Must be submitted in writing.

I, the parent, do hereby consent to participation in the above program, including all activities involved in the program. I assume all responsibilities and risks associated with my child's participation in the Ivy Bridge Academy/IvyDebate.com program.

I hereby authorize the bearer, who is a member of the staff of Ivy Bridge Academy / IvyDebate.com, to sign on my behalf any and all forms required in order to obtain emergency medical or hospital care for my child in the event that Ivy Bridge Academy is unable to contact me, and request that necessary emergency treatment be provided by you for my child. I realize that I am responsible for payment of such emergency care. A photocopy of this document shall have the same force and effect as the original.

Ivy Bridge Academy / IvyDebate.com Cancellation/Refund Policy
Year-Round Programs: To discontinue lessons, one month’s advance notice is required. Students will be given a last class date, and any class fee after that date will be refunded.
After-School Programs: Non-refundable after the first lesson day.
Summer Camp and Holiday Camp: Refundable only up to one month before the first day of camp. After this period, only tuition credits are offered.
Tournaments: No Cancellations/Refunds.
Credit Requests must be submitted in writing. Registration and handling fees are non-refundable.

Make-Up Class Policy
Year-Round Programs: Ivy Bridge requires a 24-hour advance notice before missing a class, in order to schedule a make-up class. Make-up classes can only be scheduled within the same monthly pay period. Make-up classes cannot be carried over to the next month or credited.
After-School Programs: We strive to keep class sizes small and maintain an ideal teacher/student ratio. For these reasons, we are unable to offer make-up classes.
Summer Camp and Holiday Camp: Please contact the Director of your location for information about missed classes. (Johns Creek: (404) 519-7715, Suwanee: (770) 712-2485, Marietta: (770) 712-7625)
Tournaments: No Make-Ups.

Can I switch classes/camps?
If families find that they have a scheduling conflict and need to switch their class day or time or camp, please contact us to request this change. Students will not be moved from their assigned class or camps after the credit deadline. Please be aware that switching class is contingent upon space availability in the desired class.

Is there a chance that a class could be cancelled due to low enrollment?
Ivy Bridge Academy / IvyDebate.com reserves the right to cancel classes for which enrollment does not reach the necessary level to provide the best experience for our students. In this event, we will notify the families affected by a cancelled class with a choice of either a suitable replacement classes or full refunds.

Are classes cancelled due to inclement weather?
For the safety of our students and families, Ivy Bridge Academy / IvyDebate.com reserves the right to cancel classes due to inclement weather. Make-up days for cancelled classes may or may not be scheduled based on individual class schedules and circumstances.

Late Pick Up Policy 
Each child should be picked up promptly at the appropriate time. Late pick up will result in the following charges:
* 1-5 minutes late: free
* 5-10 minutes: $10
* 10 minutes and more: $1 for each additional minute
Please note that three late pick ups will result in the student’s revocation of the right to attend the IBA After school program.

Suspension or Dismissal from After School Program Held at ES, MS, HS:
The After-School Program is a privilege offered in partnership with local schools. Each child is expected to behave properly and in accordance with code of conduct established by the schools.
Inappropriate behavior may result in the suspension or dismissal of your child from the program. Please note that no refunds can be given for suspension or dismissal from the program due to inappropriate behavior.

Absences, late arrivals, and/or withdrawals:
Families are expected to bring students to class on time. Please contact us in advance of any necessary absences or late arrivals when possible. No refunds or make up classes will be offered for absences, late arrivals, and/or withdrawals.

Lost and Found:
Be sure to label everything. Lost items may be claimed at the front desk. Ivy Bridge Academy is not responsible for any lost items.
